Thanks for your message liz. To me, akoronar just means that the aortic annulus (the anatomic annulus, not the annulus part of a valvular graft) was expanded in the non-coronary region, for example, by means of an incision and patch, prior to sewing in the pulmonary autograft. Unfortunately, my active Italian is quite poor and I cannot help with the Italian wording :-(

e ma non so se l'anulus ha una cuspide... non trovo record. Le cuspidi le ha la valvola aortica, mi risulta.

I had already fond suggestions for the meaning of "non coronary" and some occurences in Italian related to cusps, but the term seems used here as an adverb, so I don't know how to put it in the sentence.
