Mio tentativo: "Psychopharmacological nursing knowledge: comparison between medical operating and first aid psychiatry reality units." (Credo si possa omettere "reality" perché qui mi suona poco convincente).

realtà You could translate realtà in a number of ways, "life in ... units and ... units" or "how ... units and ... units operate". However, if this is your thesis, I would advise you to have it translated by a professional native English speaking translator who specialises in the medical field. Having once worked as a social worker many years ago, the concept of "first aid psychiatry" brings a smile to my face. Maybe in films.

For realtà you might want to use situation /practices but you might also do away with it altogether

"Realtà" in English is rather redundant. I would miss it out completely, and translate as "comparison between medical and acute psychiatric units".
