Give the limited context, I just about agree with Jeffrey's "self-examination", however it could be "paid-for examination" as outlined in Eodd's first reference (rather than "Self pays for medical investigation(s)"). More context could help but I know it's difficult with these medical forms!

Disagree. Medical or clinical investigation is a commonly-used term.
