"Sağlık Karnesi" is a document evidencing that the holder is covered under a health insurance plan. It is issued by any of the three public health insurance organizations, (by the Social Insurance Institution for employees working in private industry, by the Pension Fund for public officials, or by Bağkur for private Employers & self-employed businessmen)

If it is a document evidencing that students are healthy, have received certain vaccine shots, etc.
Then the answers offered by our two colleagues are more appropriate (health certificate, or certificate of health).
IF THIS IS THE CASE, PLEASE IGNORE MY ANSWER, and chose either of the two other answers that are more appropriate.
