Hi there
I have seen the document which you mentioned. I always translate them to "Half Page Document" or it could be translated to "Registration form (Short format)". I know this is a free translation but looking at those "power of attorney" which is not related to accounting and transactions, I prefer to use just "Half page document".

: dear Atena,i had written the exact meaning of "Vekalat name" ="power of attorney" just for extra explanation and i am sure about it cause i have seen it on the court room's forms.i think my main answer was just like the one Mr.Yazdunpanah has translated.
